|
본 연구는 Colaizzi (1978)의 현상학적 분석방법을 이용하여 요양병원 간호사의 도덕적 고뇌를 통해 이들이 겪는 경험에 대한 의미와 본질을 파악하였다. 9명의 연구참여자와 심층 면담을 통한 수집된 자료를 분석한 결과 122개의 의미구성, 7개의 주제모임과 3개의 범주인 ‘돌봄 의미에 대한 혼란과 고뇌’, ‘묵인에 대한 무기력함’, ‘조직에서 견뎌냄’으로 도출되었다.
본 연구는 요양병원 간호사의 도덕적 고뇌를 통한 구체적인 속성을 파악하고, 이에 대한 이해와 고찰을 바탕으로 요양병원 간호사의 전문직으로서의 도덕적 고뇌의 가치와 의미를 찾으며 자신의 직무에 대한 자부심을 위한 대책 마련을 위한 정책개발에 필요한 기초자료를 제공할 수 있다는 점에서 그 의의를 찾을 수 있다. 본 연구의 결과를 근거로 다음과 같이 제언한다. 첫째, 한국적 상황을 반영한 요양병원 간호사의 도덕적 고뇌를 측정할 수 있는 도구개발의 필요성을 제언한다. 둘째, 요양병원 간호사의 도덕적 고뇌에 영향을 미치는 관련요인을 분석하는 연구가 수행되기를 제언한다. 셋째, 요양병원 간호사의 도덕적 고뇌를 완화할 수 있는 중재 프로그램의 개발과 중재프로그램의 효과를 검증하는 연구를 후속연구로 제언한다.

Purpose: The purpose of this study was to explore the subjective experience of moral distress among nurses working in long-term care hospitals.
Methods: A phenomenological approach was used for the study. Data were collected from May to July, 2019 using open-ended questions during in-depth interviews. Participants were nurses working in long-term care hospitals and had reported experiences of moral distress. Nine nurses participated in this study.
Results: Three themes emerged from the analysis using Colaizzi’s method: (1) confusion and distress about the meaning of care, (2) a feeling of helplessness caused by connivance, (3) enduring in the organization.
Conclusion: Sufficient labor supply, environmental improvements, programs for improving interpersonal skills, education and counseling on end-of-life care, and recognition improvement about long-term care hospitals are suggested to reduce the moral distress of long-term care hospital nurses.
